The following modules and forms have been edited for RESIDENTS.

The following Orientation Modules must be completed annually by residents prior to working at a site. After you have completed all of the following modules, your residency program coordinator or heathcare facility coordinator will retain your signed and dated 'Confidentiality Agreement and Acknowledgement of Orientation Modules' form. Please give the signed/dated forms to your residency program coordinator or heathcare facility coordinator, NOT the healthcare facility.

All residents are required to meet the GGBHA immunization and health requirements (see below). Residents entering a rotation site must also complete the confidentiality agreement (see below) that includes the acknowledgement of completing and understanding the learning modules.

At the completion of a rotation experience, residents are to complete the 'Evaluation of the Rotation Site' form and return it to their residency program coordinator or heathcare facility coordinator. Faculty should submit the completed resident's 'Evaluation of the Rotation Site' forms to the healthcare facility's rotation coordinator no later than two weeks after completion of the clinical.
